As a resident of Ohio, you should take the following steps to ensure your decision to become a donor is carried out:

• Sign up online to be an organ and tissue donor with the Ohio Donor Registry at www.donatelifeohio.org.


• Sign up by filling out the Donor Registry Enrollment Form that is available at Ohio Donor Registry.


• Sign up at the Bureau of Motor Vehicles when you apply for or renew your driver's license or request a State ID card.

Ohio does not require family consent to carry out your decision to be an organ, eye or tissue donor. However, keeping everyone informed will help avoid any confusion or delays.   Print a family notification card now.
